TeacherRated (http://www.teacherrated.com) lists some amazing math sites - all have been contributed by teachers and have descriptions and indicate what grade they are appropriate for. As well, teachers can contribute additional sites and share their experience using them through ratings and reviews if they like.

You can browse by topic General Math, Geometry, Measurement, Numbers & Operations... or search by keyword, grade level etc.
